This is a single Shore Station 5" Nylatron leveling cable sheave/pulley for use with discontinued lift models:

SS-26108A
SS-36108A
SS-V26108A
SS-V36108A (both flat rack and v-rack models).

The 5" Leveling cable sheave can be found in four locations on the above lift models. These sheaves are located at the ends of the front and rear platform tubes.
